Medical Records Requests
We strive to make access to your medical records as easy as possible for the continuation of your care. To request a copy of your medical records you will need to complete an Release of Medical Information Form. Once that authorization has been received, the request will be completed and mailed to the provider as stated on the authorization within 14 days.
